Medication Consent & Senate Bill 12

Good afternoon,
Welcome back to a new school year!
I have attached a medication consent form for the medications that have been supplied by the school for administration. These medications may be administered throughout the school year and are limited to no more than three times per month. If your student needs over-the-counter medication administered more frequently, the medication must be supplied by you, and a separate consent form must be completed. I will be sending this document home with students today. Please return as soon as possible.
Additionally, I have attached the Senate Bill 12 Consent Notice, which is now required to be sent out each school year. Please review the document thoroughly. If you choose to opt out of any services, the document must be initialed, signed, and dated. If you do not choose to opt out of any health-related services, please keep the document for your reference.
If you reviewed this information and signed the medication consent form during Meet the Teacher Night, you may disregard this paperwork.
